TruHealth’s nurse practitioners (NPs) and physician assistants (PAs) are contracted to work in skilled nursing facilities in multiple states, including Tennessee, Kansas, Oklahoma and others. Our providers expand patients’ access to care and may act as care managers for specialized Medicare Advantage health plans serving residents in long-term care communities.

As a division of American Health Partners, TruHealth contributes to the company’s continuum of care and benefits from its expertise acquired over three decades. Today, American Health Partners owns nursing homes, health plans for Medicare beneficiaries living in institutions, as well as psychiatric hospitals and divisions offering home health, hospice, rehabilitation and specialty pharmacy services.

TruHealth practitioners usually work in just one or two healthcare facilities. This enables them to get to know the patients, their families and the medical team. Our clinicians’ goal is to become valuable resources throughout the communities they serve.

In long-term care settings, TruHealth practitioners collaborate with administrators, physicians and other caregivers to address patients’ medical, mental health and social needs. We train our clinicians to develop individualized care plans that combine the following proven models of care.

Holistic Care

Every person’s well-being involves body, mind and soul. This is why we seek health and behavioral interventions that fit the individual’s needs. We also work with family members and staff to create a peaceful, compassionate community.

Integrated Care

By melding primary care with mental healthcare, we create a comfortable and safe environment for patients. TruHealth providers are trained to recognize and manage mental health disorders that may occur along with other health problems.

Transitional Care

Older adults who must be transferred between care settings can benefit greatly from having a TruHealth provider assigned to oversee their quality of care every step of the way. This attention also can prevent repeated hospitalization.

We are always looking for talented individuals to join the TruHealth team. Ideal candidates will have five to eight years clinical experience, including three years as an advanced practice provider. Job responsibilities vary by state, but typically include:

  • Assessing medical or psychiatric needs
  • Managing or developing comprehensive treatment plans
  • Diagnosing and treating acute medical or psychiatric conditions
  • Prescribing and adjusting medications
  • Monitoring patient outcomes
  • Collaborating with therapists and caregivers to ensure quality care
  • Providing on-call coverage to deal with urgent medical needs
  • Educating caregivers on best practices and regulatory compliance
